Please use the attached order form in the catalog, making
sure the entire order is complete. Feel free to make copies
of the form. If you lose the form or have any further questions
regarding your order, just call us at our TOLL-FREE number.

- 9:00 a.m. to 5:00 p.m. E.S.T. - and we will be happy to
assist you. In the event that we are temporarily out of
an item that you have ordered, we will ship the available
items to you and indicate any back-ordered items on the
enclosed invoice.

order is individually packaged with the resident's name on the
outside of each bag. Individual invoices for each resident's
order will be enclosed with the order. All mail and phone orders
are processed immediately and shipped via United Parcel Service

please indicate reason for exchange, preference and a copy of
